Paracetamol (acetaminophen) is one of the most used over-the-counter analgesics and antipyretics — but it’s also the most frequent cause of acute liver failure in Australia and worldwide. While therapeutic doses are safe, overdose (intentional or unintentional) can lead to hepatotoxicity and even death if not identified and managed early.

As future pharmacists, it is essential to understand the clinical signs of paracetamol toxicity, how to assess the risk of hepatotoxicity, and when to initiate treatment. Your role may include advising on appropriate use, identifying risk in patients with high usage or chronic exposure, and supporting the management plan for overdose in hospital settings.

Key Concepts to Know Before the Activity

Toxic Dose Thresholds:
- Adults: >10 g or >200 mg/kg (whichever is lower) in a single ingestion.
- Children: >200 mg/kg in a single ingestion.
- Chronic supratherapeutic use (e.g., >4 g/day in adults for several days) can also cause toxicity.

Risk Factors for Toxicity at Lower Doses:
- Malnutrition
- Chronic alcohol use
- Liver disease
- Use of enzyme-inducing drugs (e.g., carbamazepine, phenytoin)

Toxicity Timeline:
- 0–24 hours: Often asymptomatic or nonspecific (nausea, vomiting, malaise)
- 24–72 hours: Hepatic enzyme elevation, RUQ pain, signs of liver injury
- 72–96 hours: Peak liver damage — jaundice, coagulopathy, hepatic encephalopathy
- >96 hours: Recovery phase or progression to liver failure and death

Investigations & Risk Assessment:
- Measure serum paracetamol concentration at 4 hours post-ingestion (or as soon as possible if timing unknown).
- Use the Rumack–Matthew nomogram to determine hepatotoxicity risk for single, acute overdoses.
- Monitor LFTs, INR, UEC, glucose, and ABGs as needed.

Antidote:
- N-acetylcysteine (NAC) is the antidote.
- Most effective when given within 8 hours.
- Still beneficial even if the presentation is delayed.
