This does not include a personal taste preference or a generalized dissatisfaction. Client/Customer received a different product instead of what was ordered. (i.e. ordered Golden Pancakes, but received Chocolate Chip Pancakes) Any health concern potentially related to consumption of an OPTAVIA product. OPTAVIA/Medifast bars were melted upon receipt Any other product complaint issue that cannot be defined or explained by the other product complaint categories listed in the form. Any piece of plastic, paper cloth, glass, metal, filth, insect, or any other object that does not belong in the product. The product contains fewer individual servings in the package than required. (i.e. product contains less than 7 packets, missing product baking trays, etc.) Any damage to the outside shipping box. This could be due to improper handling when loading or during delivery. Contents contain a different product from what is listed on the branded packaging.
