As it was the 1st of the month, just a day ago, the patient would have been correct. The correct answer would be “Accurate within 5 days”. The assessment has this wording in here for a reason as it may be the first few days of the month and then patient is still orienting the new month.

As it states Marvin has not eaten or had anything to drink for at least 2 years, the code for eating for this stay would be “09 Not Applicable” as Marvin did not perform the task prior to his rehab stay (for at least two years).

Wanda did not attempt the activity after she was provided cues and handed her toothbrush causing the OT to complete the task without Wanda’s effort. The correct code for this task would be dependent as the patient was unable to make any effort in completing the task.

As a helper was not present for Alexander while he managed his clothing and perineal hygiene, he would be independent with toileting hygiene in this scenario.

In this scenario, Evelyn managed her own clothing however the helper performed hygiene for her making the correct code for this Partial/Moderate Assistance.

As Joe required more than just touching assist (needing two buttons buttoned), the correct code would be Partial/Moderate Assistance.

Tammy was only able to complete putting her right arm into the shirt without assistance. She required assistance to don her abdominal binder as well as pulling her shirt over her head and threading her left arm into the shirt as well. This means that Tammy did less than 50% of the task making this code as Substantial/Maximal Assistance.

The helper assisted with donning the R compression stocking so even though Nancy donned her gripper socks with supervision, she still needed physical assistance to don the compression stocking coding her as Partial/Moderate Assistance.
