Introduction:
Chronic Kidney Disease (CKD) is a progressive, irreversible loss of kidney function that affects how the body regulates fluid, electrolytes, waste products, and medications. Pharmacists play a critical role in optimizing therapy to reduce complications, adjust dosages, and support quality of life in patients living with CKD.
Stages of CKD
CKD is classified into five stages based on estimated glomerular filtration rate (eGFR):
- Stage 1 (eGFR ≥90) –2 (eGFR 60–89): Mild or early kidney damage with normal or mildly reduced function.
- Stage 3a (eGFR 45–59) /3b (eGFR 30–44): Moderate reduction in function.
- Stage 4 (eGFR 15–29): Severe reduction in function.
- Stage 5 (eGFR <15): Kidney failure (also called end-stage kidney disease or ESKD).
As kidney function declines, the risk of complications increases — including anaemia, bone mineral disorders, fluid overload, and cardiovascular disease.
Common Medication Considerations in CKD
- Avoid nephrotoxic drugs (e.g., NSAIDs, contrast agents)
- Dose adjust medications with renal clearance (e.g., metformin, digoxin, aminoglycosides)
- Monitor electrolyte balance and avoid medications that worsen hyperkalaemia or acidosis
- Use caution with certain antihypertensives (e.g., ACE inhibitors in advanced CKD)
Common Medications Seen in CKD Patients:
- Erythropoiesis-Stimulating Agents (ESAs):
e.g. Darbepoetin alfa (Aranesp®) — used to manage anaemia of CKD due to reduced erythropoietin production.
- Phosphate Binders:
e.g. Sevelamer (Renagel®), lanthanum — to control hyperphosphataemia in CKD.
- Potassium-Binding Agents:
e.g. Sodium polystyrene sulfonate (Resonium®)— to treat hyperkalaemia.
- Active Vitamin D analogues / Calcitriol: To manage secondary hyperparathyroidism and maintain bone health.
- Sodium bicarbonate: Used in metabolic acidosis of CKD.
- Iron supplements (oral or IV):
e.g. ferric carboxymaltose (Ferinject®) — to manage iron deficiency in CKD-related anaemia.
These therapies require close monitoring of lab values (e.g. Hb, phosphate, calcium, potassium, bicarbonate) and symptom management to prevent complications.
