Drug-drug: avoid concurrent use w/MAOIs (2-4 weeks between), concurrent use w/TCA result in increased therapeutic & toxic effects, risk of serotonin syndrome if combined w/any drug/herb that increases 5HT levels (other SSRIs, St John's wort, Triptans); increased risk of bleeding if combined w/ ASA, NSAIDs, anti platelets, anticoagulants. Prototype: Chlorpromazine (Thorazine) & Clozapine (Clozaril) Contraindications: Hx of CNS depression or circulatory collapse; Parkinson's disease; coronary disease; severe hypotension, prolonged QT syndrome. AEs: Sedation, weakness, tremors, drowsiness, extrapyramidal effects. Drug-drug: Increased effects of both drugs when combined w/beta-blockers (-lol); avoid ETOH d/t increased risk of CNS depression. Prototype: Lithium (Lithobid) Contraindications: renal or cardiac disease; hx of leukemia, metabolic d/o's; dehydration & diuretic use--severe hyponatremia may occur; pregnancy/lactation. AEs: CNS problems, lethargy, slurred speech, muscle weakness, fine tremor, polyuria, gastric toxicity with N/V/D, CV collapse, coma (adverse effects related to serum drug levels. Drug-drug: Risk for encephalopathy syndrome if combined w/haloperidol. ; increased CNS toxicity if given w/carbamazipine; may block absorption of lithium if combined w/Psyllium (herbal); multiple drug-drug interactions--consult manual. Prototype: Methylphenidate (Ritalin, Concerta) Contraindications: marked anxiety/agitation/tension/fatigue/glaucoma; cardiac disease; pregnancy/lactation AEs: nervousness, insomnia, increased/decreased pulse rate & BP; tachycardia, anorexia, nausea, abdominal pain. Drug-drug interactions: Increased risk for AEs & toxicity wf combined w/MAOIs; increased risk for increased frug levels when combined w/phenytoin or TCA
