PROVIDES PLACE FOR SENIORS WHO LIVE WITH FAMILY A PLACE TO GO DURING THE DAY WHEN FAMILY IS NOT AT HOME.

CAN DIFFER FROM EXTENDED CARE FACILITIES; PLACES OF RESIDENCE FOR THOSE NEEDING CONSTANT NURSING CARE AND ASSISTANCE WITH ACTIVITIES OF DAILY LIVING (ADL'S).
A place that provides care for people who are expected to live 6 months or less.
Provide outpatient care for clients who require physical or occupational therapy, recreational therapy, hydrotherapy, and other therapies (such as speech or hearing therapy) for loss of function in mobility or the activi-ties of daily living. Patients/clients may receive prosthetics and learn how to use adaptive devices.
A PERSON WHO GOES TO A FACILITY FOR TREATMENT BUT WHO DOES NOT SPEND THE NIGHT.
Offer medical care—including diagnosis, observation, treatment, and rehabilitation—that does not require an overnight admission to a hospital or other health care facility.
OFFERS OPPORTUNITY TO REMAIN IN THEIR OWN HOME LOCATED ON A CAMPUS WITH HEALTH CARE PROFESSIONALS ON STAFF AND HAS FACILITIES AND ACTIVITIES DESIGNED FOR OLDER INDIVIDUALS.
MANAGED CARE ORGANIZATION STRESSING WELLNESS AND PREVENTION; AVOIDS UNNECESSARY MEDICAL EXPENSES; PROVIDES BASIC MEDICAL SERVICES, IMMUNIZATIONS, CHECKUPS, PATIENT EDUCATION.
Do not specialize in any one type of medical treatment; rather, they provide a wide range of diagnostic, medical, surgical, and emergency care services.
CONTINUING OR OCCURRING AGAIN AND AGAIN FOR A LONG TIME.
Provide services to predominately low-income, medically under-served individuals.
Provide inpatient continuity of care for clients with persistent, recurring diseases or complex medical conditions that require long-term stays (often over a month) in an acute care environment
